Steady support through the
new NDIS assessment process
Changes like the I CAN assessment can feel unfamiliar and, understandably, a little daunting. Rather than reacting to each change as it comes, we have built a simple three phase process with participants and care teams that is designed to remain effective as the assessment process evolves.
Identification & Collaboration
We work closely with participants, their families or guardians, and care teams to develop personalised assessments and support plans. By understanding each participant's needs and goals, we identify the evidence required to support long-term funding and informed reporting.
Support & Data Collection
Using our proprietary systems and experienced team, we deliver the supports requested by each participant while collecting the key data identified by the care team. Our staff are trained to maintain detailed notes and reports, providing the evidence needed for informed, data-driven decision-making.
Collation
The data collected is reviewed and analysed to support strategic decision-making and continuous service improvement. It is then compiled into reports for the care team and NDIS, providing the evidence needed to support participants and maintain consistent, stable plan funding.
